My Optician Does Not Show Up, How Do I Get My Optician Added?

Please forward us the following details for your optician, so we can correctly input their information into our system. Please provide us with as much information as possible about the optician that prescribes your contact lenses/eyewear. At a minimum, we would ask that you provide us with the following:

• Optician Name
• Name Of Practice
• Contact Email Address
